* Must have RN License in FL and AL. Position will support all In-patient centers in Pensacola and Mobile.

Position Summary:

The In-Patient Facility, Interdisciplinary Group RN, is a professional nurse specially trained in the Hospice concept to provide for the personal, physical, emotional and social needs of the patient/family unit. He/she works with the Interdisciplinary Group (IDG) to facilitate patient care activities; provide advice, support and assistance to the team members as needed and is responsible for ongoing patient/family evaluation and assessment. The RN is an active participant in the Hospice program including community education.


* A minimum of two (2) years of clinical nursing experience is preferred. Experience must be in a Hospice or medical/surgical setting either in a hospital, oncology or geriatric facility.

* Must have a current RN license issued by the State of Florida and Alabama.

* Must possess current CPR certification.

Main Duties and Responsibilities

A. Competently performs all basic nursing skills. (See attached "New Employee Nursing Skills List").

B. Utilizes new learning experiences to promote professional growth.

C. Acquires and maintains necessary knowledge of Hospice In-Patient Care Guidelines and documents acuity to patient level of care.

D. Maintains complete, accurate and up-to-date medical records and other records pertaining to the provision of care. Documents all contacts and has documentation in office per policy.

E. Participates every two weeks in patient care conferences.

F. Notifies Manager of all changes in status of patient as necessary. Communicates with patient's physician and Hospice Medical Director.

G. Assists other team members and ensure the best care for the patient/family according to the Hospice Concept.

H. Assists in creating and maintaining a climate that promotes growth and harmonious relationships within the Hospice staff.

I. Contributes to the development and maintenance of a written Hospice Care Plan for each patient and updates twice monthly or more often if needed.

J. Develops Hospice Aide plan of care based on patient's personal care needs.

K. Assists with food preparation for patients in the Facility.

L. Assists with necessary housekeeping and laundry chores to provide quality environment for the Facility at all times.

M. Assists Manager in maintaining health, fire and safety codes according to established policies and procedures.

N. Will be cross-trained to work at either the Joyce Goldenberg Hospice Residence and In-Patient Facility or at a hospital-based Hospice In-Patient, depending on patient census/patient care needs.

O. Assures utilization of Universal Precautions for infection control at all times.

P. Promotes and maintains excellent communication skills and working relationships with co- workers, Hospice staff, patients, families and the community at large who visit the Hospice In-Patient Facility.

Q. Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services. This applies to determinations and consultations with members of the IDG, including the Attending Physician or Hospice physician, in all circumstances in which the level of care is to be assigned as Short-term Inpatient Care or Continuous Care, and includes all actions required in Covenant's policies and procedures regarding the placement or removal of a patient from those categories of care.

R. As a member of the Interdisciplinary Group, is responsible for determining and documenting the level of care status of patients, and in particular, inpatient level of care, to ensure the patient is appropriate and meets the criteria for short-term care needed to manage pain and symptoms in an inpatient facility. This responsibility includes appropriate management and documentation with the patient's change in status either to short-term inpatient care or removed from that status, and all monitoring of the care during the inpatient period. This same requirement applies to management and documentation of patients who are placed on a Continuous Care level of care. Strict compliance with all rules and regulations, including Covenant Hospice's policies and procedures, is required.

S. Promotes a positive image of the Hospice concept throughout the community.

T. Completes Clinical Competency orientation within the 90-day probationary period.

U. Performs other duties as may be assigned by the Manager.

V. Required to comply with all Covenant Hospice Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.

W. Works effectively with volunteer services to identify patient/family needs that can be met by volunteers; offers volunteer services to patients and families, as needed, throughout their length of stay; requests volunteer services to meet identified needs; documents volunteer interventions in the Plan of Care.
