Specific Responsibilities:
A Registered Nursing License is required for this position. This is
a highly responsible Registered Nurse Specialist position
exercising independent judgment when providing health education,
support, and case management/care coordination to improve pregnancy
outcomes, child health and development and the prenatal life period
to a culturally diverse population of pregnant women, infants, and
children under 3 years of age. Maintains confidentiality regarding
all client's information. This position performs a wide range of
duties under limited supervision of the Senior Community Health
Nursing Supervisor (SCHNS). Ensures that all data set(s) under this
position's responsibilities are available to those that need to
know, protects the integrity of those assigned data set(s), and
assists the Security Coordinator as needed to comply with policies
and procedures described in Security Policy 3 - area of
responsibilities. Travel may be required.
Responsible for proving intense case management/care coordination
and outreach activities to a large high-risk complex Healthy Start
case load according to the regulations of the Healthy Start
Standards and Guidelines. Activities include scheduling
appointments, making telephone calls to clients and health care
providers, conducting required home visits to complete Healthy
Start pathways, prenatal and postnatal comprehensive assessments,
care coordination services, completing infants and toddlers Age and
Stages Questionnaires, and initiating referrals for those children
that are not meeting developmental milestones and be able to work
with clients of diverse backgrounds and values. Provides community
referrals based upon the needs of the client and family. Assesses
and evaluates the clients' Individual Plan of Care with each client
encounter, assists clients in making physicians' appointments.
Responsible for developing, updating, and coordinating a Family
Support Plan for each Healthy Start family. Care coordination
services also includes outreach and consultation with community
agencies and physicians to minimize obstacles in accessing
services. Documents and codes all services based upon Healthy Start
Standards and Guidelines and established protocols in the client
electronic record.
In-Services / Staff Development: Participates in in services and
staff development as it relates to job performance.
Other Related Duties: Perform other related duties as assigned.
Required Knowledge, Skills, and Abilities:
Knowledge of the principles and practices of nursing and skills in
the application of nursing in Maternal Child Health.
Ability to plan, organize and coordinate large work
assignments.
Ability to provide casework services to clients.
Ability to demonstrates competency in using electronic health
records, evidence based educational curriculums, and Microsoft
Office
Applications (Outlook, Word, Microsoft Teams, and OneDrive).
Qualifications:
Minimum - Registered Professional Nursing license in accordance
with Chapter 484 Florida Statues and in accordance with Rule 64B9,
Florida Administrative Code. At least 1 to 3 years of Experience. A
Valid Driver's License.
