Job Description:

LPN Clinical Care Coordinator needed for the Pensacola area!
Schedule: Monday - Thursday, 8am - 4:30pm and Friday 8am - 12pm
No travel or float requirements.
The position is for professional nursing care delivered to patients following established medical standards and practices, and as such, requires emphasis on excellent patient care and achieving superior clinical outcomes with a patient-centric care experience. The Coordinator will assist RN and APRN staff with treatment cycle management. Responsibilities span diverse aspects of clinical infertility care, including: management of new patient intake, records requests/documentation, scheduling, reporting, documentation of diagnostic testing, and assisting with the management of findings. Responsibilities also include implementation and documentation of treatment, medication management, patient education and care.
Education/Qualifications:

• Certificate/License, required: Possession of a valid, unexpired Florida LPN license.
  Education/Qualifications:
  • Licensed Practical Nurse (LPN): Graduate of an accredited school of nursing.
  • Excellent oral and written communication skills needed, strong organizational ability required.
    Essential duties, including but not limited to:
    • Responsibilities span diverse aspects of clinical infertility care, including: management of new patient intake, records requests/documentation, scheduling, reporting, documentation of diagnostic testing, and assisting with the management of findings.
    • Implementation and documentation of treatment, medication management, patient education and care.
    • Assist providers daily with patient care coordination.
    • Treatment plan coordination. Ensure testing summary for diagnostic testing is up to date and complete.
    • Perform patient vital signs, phlebotomy and other required tests, record patient history and obtain any information the physician may request to enable him/her to assist in the diagnosis and treatment of the patient.
    • Schedule and process paperwork for patient tests, procedures, labs, medications, office appointments or procedures etc.
    • Initiate prescription orders via the electronic medical record or fax.
    • Ensure exam rooms are clean, stocked appropriately and ready for next patient; ensure equipment is clean/sterile and in working order.
    • All other duties as assigned.
