Position Overview The primary responsibility of the Hospice Care Hospital Liaison is to provide navigation services to assist key hospital staff in identifying patients appropriate for hospice care, assess new referrals for hospice appropriateness, provide recommendations for alternatives for care if the patient is not eligible for hospice services, provide education to key hospital staff, patients and families on hospice care, admit hospice appropriate patients and ease the transition between the hospital setting and the next location of care. The hospice care hospital liaison will perform initial consultation visit to explain Covenant services and establish appropriateness for hospice and conduct initial physical assessment of patient for the purpose of identification of immediate problems and follow-up as necessary. Communicate to patient/family a thorough introduction to Hospice philosophy and services available. Obtain Consent for Care and appropriate election statements as required by payer source. Visit daily and/or assess any patient admitted to assigned hospital under a short-term inpatient level of care and document status and level of care justification in the EMR. Work closely with the Provider Relations Representative to provide education, support and collateral materials to the staff of assigned facilities. Collaborate actively with Care Navigation Center for the purpose of facilitating admission to Covenant Hospice care. Ensure smooth transition among healthcare settings. Minimum Qualifications: Registered Nurse must have minimum of (2) years of clinical nursing experience. Community education experience and case management experience is desired. Nurse must be a graduate of an accredited school of nursing and have a current license in the State of Florida or Alabama, as applicable. A Bachelor's Degree or higher is highly desired. Experience must be in medical/surgical or oncology nursing, preferably with Hospice experience. Significant hospice experience and related healthcare education and clinical experience are highly desired. Candidate must possess strong interpersonal skills and be proactive in resolving challenging situations. To avoid any potential conflict of interest and to ensure regulatory compliance, Nurse Navigators may not be employed by or as a consultant to any other employer while also employed at Covenant Hospice, without prior approval by the incumbent's supervisor and then the Vice-President of Human Resources. Must possess a valid Florida or Alabama driver's license, as applicable, depending on assignment, and current automobile liability insurance and insurability under the company liability insurance are required. Must possess current CPR certification. Must have current CHPN credentials or obtain it within 1 year of hire/promotion.
