Summary: The Spiritual Support Chaplain is responsible for
Spiritual Community outreach and clergy counseling service. He/she
will establish and maintain working relationships with the other
disciplines, employees, families, and in particular community
agencies. He/she will be available to compliment the patient/family
clergy when appropriate but may become primary clergy if
patient/family has such a need. He/she will make business
development visits to area communities and providers to give
education on Covenant Care, Grief and Counseling options available.
Responsible for providing bereavement services to grieving
individuals through individual and group support, home visits,
personal telephone and mail contact, grief workshops or seminars,
resource information, and referrals to other agencies.
Qualifications: Be an ordained minister in good standing with his
or her denomination. Possess a Master of Divinity Degree from an
accredited seminary, or a document that certifies that he or she
has completed formal training in his or her religious or belief
system and is qualified to perform pastoral services with five (5)
years of pastoral experience. Previous experience in counseling
with pediatrics and adults in the grief process. Previous sales
experience is not required but would be a positive asset to the
position. He or she should be able to function independently with
flexibility and personal integrity. He or she must have an
understanding and openness to all belief systems. Must be
knowledgeable in areas of death, dying, grief and loss issues,
group facilitation and public speaking. Experience in working with
terminally ill patients and bereaved families is preferred. Must
possess valid Florida and/or Alabama driver's license and current
automobile insurance and insurability under the company liability
insurance are required. Main Duties and Responsibilities
Demonstrates understanding of grief issues and needs and end of
life care planning. Demonstrates good assessment skills in
spiritual assessment and in planning spiritual care. Assesses
spiritual needs of patient/family unit as requested. Evaluates
presence of supportive spiritual network and identifies unmet
spiritual needs. Assesses adjustment to illness. Assist
patients/families with funeral planning. Demonstrates understanding
of the spiritual and bereavement aspects of the terminally ill
patient/family. Demonstrates knowledge of and respect for different
faith systems. Identifies and addresses spiritual distress and
intervenes appropriately. Demonstrates knowledge of and displays
good intervention skills regarding spiritual aspects of hospice
care. Demonstrates sensitivity to issues of diversity (cultural,
sexual, age, and lifestyle (etc.). The Spiritual Care Counselor
while representing Covenant Care in public will provide speeches
and prayers that are non-sectarian and interfaith in content.
Exceptions to this rule will be limited to funerals, memorial
services and clinical interventions that are requested by the
patient/family. Displays ability to communicate well in sensitive
areas. Develops, organizes, and facilitates bereavement support
groups. Plans, organizes and facilitates Remembrance Celebrations
and other special programs. Demonstrates understanding of medical
and terminal illness, including related symptomatology and
procedures. Able to explain Standard Precautions and other relevant
infection control techniques. Able to discuss pain and symptom
control. Participates in the IDG and acts as a support/resource
person to team members. Advises and consults with nurses, clergy,
hospice volunteers, and other related Documentation reflects and
supports the care plan. The care plan is updated as changes occur
to the patients condition in a timely manner and communicated to
other members of the team via IDG. Serves as liaison to community
churches, clergy, and religious organizations and makes referrals
to appropriate organizations on behalf of the patient and family.
Possesses thorough understanding of contract care to provide
Spiritual Care Counselor services for patients/families in
hospitals, nursing homes, and adult living facilities. Completes
all documentation in a timely manner. Collaborates with team
members to establish initial Interdisciplinary Group Plan of Care.
Collaborates with business development in ways to integrate
Covenant Care Completes the Spiritual Care Counselor progress note
within 48 hours of visit or telephone contact. Assumes primary
responsibility for own professional development. Completes
orientation activities within the 90-day probation period. Provides
Human Resources with copy of ordination certificate and appropriate
diplomas. Updates skills through participation in seminars,
workshops, etc. Attends staff meetings. Participates in volunteer
training and community education activities as requested. Assists
in the review and development of hospice policies in regard to
Spiritual Care Counselor services. Promotes a positive image of
Covenant Care throughout the community. Perform other duties as may
be assigned by the Regional Director of Operations. Required to
comply with all Covenant Hospice Policies, Procedures and Personnel
Policies, including those specifically addressing disaster
preparedness and response. Documents to the patient's terminality,
and establishes and maintains the care plan for spiritual support
services. This applies to determinations and consultations with
members of the IDG, including the Attending Physician or Hospice
physician, in all circumstances in which the level of care is to be
assigned as Short-term Inpatient Care or Continuous Care, and
includes all actions required in Covenant's policies and procedures
regarding the placement or removal of a patient from those
categories of care. As a member of the Interdisciplinary Group, is
responsible for documenting the level of care status of patients,
and in particular, inpatient level of care, to ensure the patient
is appropriate and meets the criteria for short-term care needed to
manage pain and symptoms in an inpatient facility. This
responsibility includes appropriate management and documentation
with the patient's change in status either to short-term inpatient
care or removed from that status, and all monitoring of the care
during the inpatient period. This same requirement applies to
management and documentation of patients who are placed on a
Continuous Care level of care. Strict compliance with all rules and
regulations, including Covenant Hospice's policies and procedures,
is required. Works effectively with volunteer and business
development services to identify patient/family needs that can be
met by volunteers; offers volunteer services to patients and
families, as needed, throughout their length of stay; requests
volunteer services to meet identified needs; documents volunteer
interventions in the Plan of Care. Special Demands/Physical
Requirements: This position may sometimes require light/medium
lifting. This position is also mentally demanding because of the
continual need for interaction of patents and families. In
addition, this position requires considerable time traveling in a
motor vehicle in connection with performing essential functions of
