Position Summary:The primary function of the Review Choice Demonstration (RCD) Case Manager is to serve as final review for the RCD submission process. To ensure quality, and to coordinate with the parent office interdisciplinary team and all branches.Essential Job Functions:Reviews and ensures quality and accuracy of all Task Attachments required for RCD submissionsAccountable for non-affirmed patient percentages-based Face to Face (F2F) insufficiency and homebound statusSubmission of all RCD required submissions through eServices, eSolutions, or HCHB portal for assigned geographyAccountable for an established minimal productivity for the number of submissions per day, per week, and per monthCommunicate all RCD submission decisions and follow-up that is required to local management for the assigned geographyFollow-up with local management pertaining to the document or task improvement required for RCD resubmission versus patient discharge preparation for the non-affirmed patientsCollaborate with the Vice President of Operations and Vice President Clinical for tracking the percentage of non-affirmed patients and categorize them by the specific reasons for the assigned geographyCollaborate with the Vice President of Operations and Vice President Clinical in order to guide regional director teams about the specific local office educational needs based on patient non-affirmationsRemain current with Brightree software updates to ensure maximization of the software utilization for the RCD demonstrationOther duties, as assignedSkills, Abilities, and Experience:Minimum 2 years home health care experience as Registered Nurse.Knowledge of the Conditions of Participation for Home Health including but not limited to:Physician Face to Face encounter for home health patientsPhysicians order for home health servicesComplete understanding of the documentation required to support homebound status for patients, patients having a medical diagnoses that are supported by a home health plan of care, and the ability to identify the ongoing need for intermediate skilled nursing, physical therapy, speech therapy, or the continuation of occupational therapyAbility to communicate effectively and teach clinical/operational staff about home health regulations,Detail oriented and able to work with minimal supervision and oversightMust be computer literateKnowledge of Brightree a plus, but not required Contact the Human Resources Department at 850.365.9659 for more information.
