Summary:

The Remote Patient Monitoring (RPM) LPN is a nurse specially trained in Home Health, Hospice and/or Palliative Care. He/she works with the clinical care teams to facilitate deployment and monitoring of RPM equipment. LPN is responsible for centralized monitoring station, management of incoming data and patient outreach and follow up. LPN will provide support and assistance to the team members as needed and is responsible for ongoing patient/family evaluation, observation, support and education.

Qualifications:

  • A minimum of two (2) years of clinical nursing experience with independent critical thinking skills.

  • A current LPN license in the State of Florida.

  • Experience must include clerical as well as computer proficiency.

  • Must possess excellent communication and interpersonal skills.

  • Reliable transportation, valid state of Florida driver's license and current automobile liability insurance and insurability under the company liability insurance are required.

  • Must possess current CPR certification.

Main Duties and Responsibilities

Responsible for centralized monitoring station for patients on RPM

Responds to risk alerts timely by use of text, phone or video chats. Uses nursing assessment skills and appropriate questions to accurately evaluate problems/needs. Collaborates with field clinicians to coordinate care and assessment of patient needs. Maintains effective communication by consulting with clinicians, managers or practitioners to assist with care plan changes and status updates or changes. Responsible for providing Performance Improvement support Monitors for program compliance and updates clinical team as issues arise. Analyzes daily monitoring data for opportunities to improve care and performance of RPM equipment.

Inventory Management

  1. Maintains appropriate inventory stock as determined by Program Director.

  2. Allocates equipment to appropriate locations and assigns to subgroups.

  3. Ensures equipment is charged and necessary peripherals are included prior to deployment to patient.

  4. Manages repair and replacement equipment with Support Team as needed.

  5. Manages equipment cleaning and disinfection upon discontinuation.

Telemonitoring Setup and Education

  1. Setting up tablet and peripheral devices in patient's home.

  2. Educating patient/family on telehealth equipment and use.

  3. Educating field clinicians on deployment processes, device usage, monitoring practices and tracking methods.

  4. Completes Clinical Competency orientation within the 90-day probation period.

  5. Demonstrates self-directed behavior.

  6. Promotes a positive image of Covenant Care throughout the community.

  7. Participates in community projects if possible.

  8. Demonstrates appropriate dress and grooming.

  9. Uses chain of command for problem solving.

  10. Displays courtesy, tact, and patience.

  11. Maintains a good attendance record.

  12. Notifies agency of absences according to policy.

  13. Keeps neat and legible written documents.

  14. Participates in agency Q/A as appropriate.

  15. Attends mandatory general staff meetings, and other meetings as requested by supervisor.

Required to comply with all Covenant Hospice Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.
